Some school districts restrict access to Google Forms that include an upload link. For this reason, we are offering two alternative procedures for submitting your application. The first is to use the button, below, that links to a Google Form that is less likely to be blocked. This is the preferred method as it is more efficient for you and provides you with a report summarizing your application. Alternatively, you can complete the MO SW-PBS Award of Excellence Worksheet on pages 7-11 of the MO SW-PBS Award of Excellence Application Guide. You will then need to scan the document and submit by email to email@example.com. Please include up to 3 pictures that captures your SW-PBS implementation. These pictures will be used during opening ceremonies at the 2020 MO SW-PBS Summer Institute (all necessary permissions and waivers for public use of photographs are the responsibility of the school or district).
