Every year, MO SW-PBS makes available to our partner schools an End of Year (EoY) report summarizing annual implementation fidelity and student outcome data for the year that is ending. Comparing the actions of adults to the impact on students can be invaluable as you begin the process of setting school improvement goals for the coming school year.

Obviously, this report is only as good as the data that has been submitted. While we pull together much of the implementation fidelity data for you, we do not have access to data regarding important student outcomes. We rely on you to provide this data for us. MO SW-PBS team members should look for an email with a  link you can use to enter  this  data  directly. You will have access to your EoY report on submit.

Student Outcome Description
Student Assistance Referrals (by grade level) Referrals for students who need additional supports  in order  to succeed in  school. This includes, but is  not  limited to, referrals for Tier 2 and  Tier  3  academic  and  behavioral supports. In addition, it  includes  referrals  to  outside agencies, such as social/emotional/mental health services, physical  health referrals, food assistance, etc.
Special Education (by grade level) Number  of  students referred  for academic or  behavioral special education
Special Education Eligible (by grade level) Number of students determined to be  eligible for special education
Outside Agencies List  the  agencies to which students have been  referred (i.e., Community Mental  Health  Services, Harvesters, etc.)
Students with IEPs who  have ODRs (by grade level) Number  of students with IEPs per grade level who have one or  more ODRs.
Non-IEP Students with ODRs (by grade level) Number  of students  receiving general education  services who have one or more  ODRs.
 0-1 ODRs  The number of students who are  responding  to Tier  1/Universal  interventions.
 2-5 ODRs  The number of  students  who may meet decision rule for  a Tier 2/Targeted  intervention.
 6 or more ODRs  The number  of students who may meet decision rule for a Tier  3/Intensive intervention.
 Total students enrolled  Used  to calculate percentages of  students with 0-1, 2-5, or 6+ ODRs
