Missouri Schoolwide Positive Beahvior Support: New Teams Commitment Resources

School teams that are considering committing to the Schoolwide Positive Behavior Support (SW-PBS) initiative in the state of Missouri have several steps in the application process. The following documents are intended to help guide such teams through the application process:

Checklist for Application A handy guide for teams to use to ensure that all forms are completed and included in the application
Administrator Questionnaire A short questionnaire for administrative reflection
Pre Overview Survey for ENTIRE staff A survey for the entire staff to take to guide overview topics/discussions
Inital School Readiness Checklist A short checklist to guide readiness activities at the school
New School Commitment Sheet (Online version of form fill-in the blanks and print out the page)
School Team Form A form to ensure that the newly forming SW-PBS team will be representative of staff
Training and Meeting Agreement An outline of the upcoming year's training expectations for and commitment from the new SW-PBS team
PBS Survey Account Application An application from for an online account with the National PBS Survey site for ease of data collection and report generation to support decision making at the building level
